Diane, our licencing secretary, has informed me that she is receiving incorrectly completed forms.

Please make sure that you read the instructions and fill in every section that is required, also keep the pink sheet for your own records and the yellow one should be returned to Lisa.

If this is not done correctly you will not get your licence returned.
